What to Bring on Your First Visit

We ask that you bring your completed new patient paperwork (listed below) and your dental insurance information to your first visit. If you prefer to complete the new Patient Paperwork in the office, please arrive 15 minutes early for your appointment.

You might also want to write down any questions or concerns about your oral health and bring this list in with you; this will help ensure that all your concerns are addressed.

What to Expect on Your First Visit

Your first visit will include the following:

  • A review of your complete dental and medical history form
  • A review of your dental X-rays and/or taking new X-rays as required
  • A dental exam

After a thorough exam, Dr. Yurovsky will discuss her findings and, if necessary, all options for treatment. In this discussion, her goal is to help you make an informed decision regarding your dental health.

Dental Insurance

We accept all private insurance plans and help you determine what portion of your treatment is covered by insurance. Please bring your insurance card or papers on the first visit.

However, please understand that you are ultimately responsible for the payment of all services. Should your insurance not fully cover treatment, you will be responsible for paying any uncovered portion of the bill.

Payment Options

Our knowledgeable staff will work with you to ensure that any portion of the treatment not covered by insurance is affordable. We accept all major credit cards (MasterCard, Visa, Discover, American Express).
